The CPAP Compliance Problem

CPAP therapy is the gold standard treatment for obstructive sleep apnea. It works — when patients use it. The problem? Nearly 50% of patients abandon CPAP within the first year.

For providers, this means untreated sleep apnea, increased cardiovascular risk, and patients who cycle back through your office with the same complaints. For suppliers, it means returns, re-orders, and wasted resources.

The solution starts with understanding why patients quit — and building systems to catch them before they do.

Why Patients Stop Using CPAP

The top reasons for CPAP non-compliance:

  1. Mask discomfort (35%) — Wrong size, wrong style, or poor fit
  2. Nasal congestion/dryness (25%) — Inadequate humidification
  3. Claustrophobia (15%) — Often addressable with mask style changes
  4. Noise (10%) — Older machines or mask leaks
  5. Inconvenience (15%) — Travel, lifestyle factors, partner complaints

Notice: most of these are fixable equipment issues, not patient attitude problems.

Medicare’s Compliance Requirements

Medicare has specific adherence requirements for continued CPAP coverage:

  • First 90 days: The patient must use the CPAP for at least 4 hours per night on 70% of nights (roughly 21 out of 30 nights)
  • Face-to-face visit: The prescribing physician must see the patient between days 31 and 90 to assess compliance and clinical improvement
  • Data download: Usage data from the CPAP machine must be reviewed and documented

If the patient fails to meet these requirements, Medicare will not cover the equipment beyond the initial period. The machine must be returned.

What This Means for Providers

Schedule that follow-up visit early. Don’t wait until day 85 to check compliance — by then it’s too late to intervene. We recommend a check-in at day 30 and a formal visit by day 60.

Key insight: Patients who replace supplies on schedule are significantly more likely to stay compliant. Old, worn masks leak, reducing therapy effectiveness and comfort. A proactive resupply program is one of the best compliance tools available.

  • Set expectations early — Explain the 90-day compliance window at the initial prescription
  • Schedule proactive follow-ups — Day 30 phone check, day 60 in-person visit
  • Address mask issues immediately — A quick mask refit is cheaper than losing the patient
  • Review compliance data — Modern CPAP machines transmit data wirelessly. Use it.

For Suppliers

  • First-night support — Walk the patient through setup, fitting, and what to expect
  • 48-hour follow-up call — Check in after the first two nights. This is when most patients struggle.
